Requirements:

The form that this tool generates takes full advantage of ASP.NET  2.0 (the same technology required for Master Pages and most of the other cool stuff  in Expression Web). The generated form and script must be placed in an ASP.NET page (.aspx) and deployed to a windows web server that supports ASP.NET 2.0 (not just ASP or ASP.NET or ASP.NET 1.1, but ASP.NET 2.0). No FrontPage  Server Extensions required.

Step 1 - Form Fields

Select your desired form fields. Don't worry if it doesn't match your needs perfectly; you can add, edit, and remove fields later in Expression  Web without having to modify the script!
